St James Nursing and Rehabilitation Center
This is a 150 Medicare certified bed building with additional eight chairs dialysis center. The building has three floors. On the first floor, there are 25
beds, one nurse and two Certified Nurse assistants on all three shifts. Most of the patients on the first floor are one to two person assist, mainly continent of both bowel and bladder. The second floor holds 50 beds, with two licensed nurses and four certified nurse assistants on all three shifts.
The second floor is mainly a skilled floor. The third floor is mixed with long term, skilled, dementia and Alzheimer patients. This floor holds 60
patients with two nurses and five certified nurse assistants on all shift.
There are five departments:
Administrative department, head by the administrator. In this department, there are two units Secretaries, three receptionists, medical records,
Supplies and Scheduler.
Nursing Department: This department is headed by the Director of Nursing, assisted Assistant Director of Nursing, Clinical Nurse Manager, Second
shift supervisor, Weekends supervisors, Wound care nurses, floor nurses and the certified nurse assistants.
Admission Department: This department is head by the admission director
Social Service Department: This department is head by the Social service director and one assistant
Activities department: This department is head by the Activities director, assisted by two activities aides
Maintenance department; This department is head by the director of maintenance and assisted one maintenance assistance
The building outsourced the Dietary, Housekeeping, Laundry, Dialysis, and the Therapy departments
